is it possible to manually change the title, for example I have Elder Veterinarian?

It will automatically show the highest skill as title unless you have more than one equal skill. If they are maxxed you can control the skill title that displays by locking all but the one you want to show. For instance if you are maxxed at 100 swords and 100 magery and they are your highest skills if you lock sword it will show GM Mage if you lock magery it will show GM Swordsman.
